Praying Mantis And Fruit Fly Order Form

My daughter recently received the Praying Mantis Encounter (product number 767715) as a gift, and in order to get the actual praying mantis egg case, we had to fill out the form that was enclosed, and mail it in with payment. We opted for the $4.95 single egg case.

But prior to mailing in the form, I though I'd save some time by finding a place online that I could purchase it from. But the cheapest place I found wanted $8.00 plus $6.00 shipping = $14.00. So instead, I have scanned our order form and placed it here for your convenience. So if you want cheaper praying mantis egg cases and fruit flies, just download the form I scanned, print it out, fill it out, and mail it in to the address on the form!
